The Lifeline Test is covered by many major insurance companies. When your doctor requests your test, your insurance company will be able to tell you how much they will reimburse you for your test. Just give them the diagnostic code number on the front page of your physician’s requisition form. If your co-payment is too high for you to pay, or if you are paying cash, and you would like a payment plan, please email us and we will do our best to make your test available for you. After you receive your test results, please use your physician requisition form to submit your claim for reimbursement. Email us if you need assistance.
If you would like to know how much your insurance company will reimburse you before you order your test, email us and we will send you a list of the codes most often used by physicians when ordering our test.
United Health requires pre-authorization before you order your test. Other companies may also. You may wish to inquire before ordering.
